What Benefits Does Composites One Offer?

Hospital / Major Medical Plan Coverage

Eligibility - Regular full-time non-union employees are eligible after 90 days of full time, active employment and upon completion of enrollment form.

Coverage:

  • Composites One LLC currently provides medical coverage at a cost of $479.70 annually ($9.23 per weekly pay period/ $18.45 per bi-weekly pay period to its eligible employees.)
  • Coverage for eligible employee and dependents is offered at a cost of $ 1,555.06 annually ($29.91 per weekly pay period/ $59.81 per bi-weekly pay period) Employee premiums are payroll deducted on a pre-tax basis. (Appropriate forms must be completed.)
  • Coverage elections can only be changed at annual open enrollment unless employee has a qualifying life event.(i.e. divorce, marriage, birth)
  • The plan is “PPO” based for providers and hospital benefits at 90% coverage with no deductible. Certain “Wellness” benefits are covered at 100% in network
  • Out of Network provider and hospital coverage is 60% and subject to annual deductible of $300 per person or $900.00/family. Plus $250 per use hospital deductible
  • Prescription cards with mandatory generic use. Mail order available for maintenance medication (90 day supply).
  • Coverage is subject to pre-existing clause. Conditions treated in the 90-day period prior to coverage will be excluded from coverage under the plan for 12 months.
  • Pre-existing limitations can be reduced or eliminated by presenting a "HIPPA" letter which details prior medical coverage. Your pre-existing limitation can be reduced by the amount of previous continuous (coverage with less than a sixty-three day break) coverage. Your ninety-day waiting period for coverage under the company group plan will not count as a break in coverage.

Vision Plan

  • Annual exams covered at 100%
  • Frames & lenses = 20% discount
  • Contact lenses = 15% discount
  • Coverage linked to medical plan and only available to those enrolled in Medical Plan, provided at no cost to employee.
  • Discount available for in-network provider laser surgery.

Dental Plan Coverage

Eligibility - Regular full-time non-union employees who elect coverage upon completion of enrollment form and 90 days of active employment .

Coverage:

  • Composites One LLC offers dental coverage for eligible employees and dependents, on a contributory basis. Employee premiums are payroll deducted on a pre-tax basis.
  • Annual cost for single coverage is $53.30, family coverage is $172.90
  • Free exams and cleaning twice yearly.
  • Most basic and major dental services covered.
  • Orthodontic services coverage to age 19.
  • Freedom of choice in selecting dentist.
  • Maximum benefit $1,500 per calendar year.

Life / AD&D Plan Coverage

Eligibility - Regular full-time non-union employees upon completion of enrollment form and 90 days of active employment.

Coverage:

  • provides life insurance in the amount of two times the current annual base salary to its eligible employees.
  • provides an Accidental Death & Dismemberment clause as part of the life insurance coverage.
  • additional Life/AD&D Insurance or dependent coverage is not available.

Composites One LLC Profit Sharing and Savings Plan

The Profit Sharing and Savings Plan is offered to help employees prepare for a financially sound retirement. Eligible, non-union employees participate in the company Profit Sharing Plan, and have the option of enrolling in the 401k portion on a pre-tax and post-tax basis.

Eligibility – Profit Sharing: All regular, full-time non-union and regular, part-time non-union employees automatically become eligible to participate on the January 1st following their date of employment.

At year end, based upon the decision by the Board of Directors the company may make an additional discretionary profit sharing contribution to all eligible employees.

Eligibility – 401k: All regular, full-time non-union and regular, part-time non-union employees are eligible to participate and will be automatically enrolled for a minimum deferral of 4% on their date of employment. Employees who do not wish to participate must sign a deferral waiver.

Eligible employees can contribute up to a possible maximum of 20% of eligible compensation based on total participation. The company match is currently 50 cents on the dollar up to 4% of eligible compensation.

Employees are offered a choice of investment vehicles.

Employees are always 100% vested in their own 401k contributions and company match. Profit sharing funds vest over a period of 6 years.

Safety Shoes/Glasses

Safety shoes and safety glasses are required in the QC & Product Development Labs, Production and Warehouse areas. The company will cover the cost of Safety shoes and the additional cost to upgrade prescription lenses to safety lenses for eligible employees. (See Your supervisor for full details of Policy)

Holidays

Composites One LLC usually observes the following 9 paid holidays on an annual basis for full time and regular part time employees. This list is subject to change annually depending on the calendar occurrence of the holiday and business needs.

Labor Day 1 Floating Holiday (assigned annually by the Company)

Vacation

For regular full time employees, vacation will be earned and used on a “current year” calendar basis, according to the following schedule.

First Year of Service

Hired prior to June 30 4 hrs for each complete month worked during the first calendar year.
Hired after June 30   None during the first calendar year

Subsequent Years

1 through 5 years 2 weeks
6 through 15 years   3 weeks
15 or more   4 weeks

Regular part time employees earn vacation on the same schedule but time is prorated according to the number of hours actually worked.

Sick/Emergency Days

For regular full time employees, Composites One LLC will provide a bank of five days annually to be used for illness or personal emergencies. Any unused time is not paid or carried forward.

Leaves

Once requirements are met, Composites One LLC will grant approved time off in the form of leaves for the following reasons:

Military Leave up to 2 weeks annually for reserve training or active duty leave
Funeral Leave   3 days for designated family members
Family and Medical Leave   12 weeks annually

Flexible Spending Account

Maximums/Minimums
Health Care Flexible Spending Account (HFSA)

$2,000 maximum annual contribution
$300   minimum annual contribution

Dependent Care Flexible Spending Account (DFSA)

$5,000 maximum annual contribution (regardless of the number of dependents)
$2,500   maximum annual contribution for married filing seperately
$300   minimum annual contribution

Reimbursement

Claims received are paid on a bi-weekly basis either by check or direct deposit

HFSA and DFSA are separate accounts. Funds cannot be transferred between accounts. Expenses must be incurred during the period you are eligible to participate in the plan year. An expense is considered incurred on the date when the service is received, not the date service is billed or paid for.

Grace Periods

You have 90 days following the end of the plan year to submit claims for expenses incurred during the plan year.

You have 90 days from your date of termination to submit claims for services incurred while you were participating.

Eligibility

You must be a full-time non-union employee working at least 30 hours per week for a period of 90 days. Eligible employees may participate on the date the employee becomes eligible.

Termination

Your FSA will terminate on the earliest of the following dates:

  • The date you terminate employment or otherwise cease to be eligible to participate as described under elegibility.
  • The date the plan is terminated

Upon termination of the FSA, you will have a grace period to submit claims for reimbursement for expenses incurred before your termination.

If you are participating in an HFSA at the time of a COBRA qualifying event, you may elect to continue your coverage in that account under the federally mandated continuation (COBRA), if applicable to your employer. Contact your benefits administrator for additional information concerning when, and if, coverage may be continued. If HFSA is continued under COBRA, charges incurred during the period of continued coverage would be eligible for reimbursement as long as the participant continues to contribute to the HFSA.
