What Benefits Does Composites One Offer?

Hospital / Major Medical Plan Coverage

Eligibility - Regular full-time non-union employees are eligible after 90 days of full time, active employment and upon completion of enrollment form.

Coverage:

  • Great Lakes Synergy/C1 currently provides medical coverage for employees on a 4-tier structure with higher annual premiums for tobacco users and spouses who are eligible for insurance elsewhere but do not elect that insurance (spousal surcharges); premiums are deducted per pay period on a pre-tax basis.
  • Coverage elections can only be changed at annual open enrollment unless employee has a qualifying life event. (i.e. divorce, marriage, birth)
  • In Network coverage (Cigna’s Open Access Plus Network) is 90% with no annual deductible.
  • Out of Network provider coverage is 60% and subject to annual deductible of $300.00/individual or $900.00/family.
  • Pre-existing limitations can be reduced or eliminated by presenting a "HIPPA" letter which details prior medical coverage. Your pre-existing limitation can be reduced by the amount of previous continuous (coverage with less than a sixty-three day break) coverage. Your ninety-day waiting period for coverage under the company group plan will not count as a break in coverage.

Prescription Plan

  • Retail Co-Pays for 30-day supply are $10 for generic, $25 for brand, $40 for non-preferred.
  • Mail Order Maintenance Medication available Co-Pays for 90-day supply are $25 for generic, $63 for brand, and $100 for non-preferred.

Vision Plan

  • Annual exams covered at 100%
  • Frames & lenses = 20% discount
  • Contact lenses = 15% discount
  • Coverage linked to medical plan and only available to those enrolled in Medical Plan, provided at no cost to employee.
  • Discount available for in-network provider laser surgery.

Dental Plan Coverage

Eligibility - Regular full-time non-union employees who elect coverage upon completion of enrollment form and 90 days of active employment .

Coverage:

  • Great Lakes Synergy/C1 currently provides dental coverage for employees on a 4-tier structure with higher annual premiums for tobacco users; premiums are deducted per pay period on a pre-tax basis.
  • Free exams and cleaning twice yearly.
  • Most basic and major dental services covered.
  • Orthodontic services coverage to age 19.
  • Freedom of choice in selecting dentist.
  • Maximum benefit $1,500 per calendar year.

Life / AD&D Plan Coverage

Eligibility - Regular full-time non-union employees upon completion of enrollment form and 90 days of active employment.

Coverage:

  • The company provides life insurance in the amount of two times the current annual base salary to its eligible employees.
  • The company provides an Accidental Death & Dismemberment clause as part of the life insurance coverage.

  • Educational Assistance Program, refer to employee handbook
  • Employee Assistance program (EAP), Confidential service to help our employees and their legal dependents handle personal, legal, financial or work related problems. Refer to employee benefits handbook or Company Intranet.
  • Optional Life Insurance, The Company provides optional life insurance for you, spouse and children. Evidence of Insurability is required for insurance over specific amounts.

Composites One LLC Profit Sharing and Savings Plan

The Profit Sharing and Savings Plan is offered by Great Lakes Synergy to help employees prepare for a financially sound retirement.  Eligible, non-union Great Lakes Synergy employees participate in the company Profit Sharing Plan, and have the option of enrolling in the 401(k) portion on a pre-tax and post-tax basis.

EligibilityProfit Sharing: All regular, full-time non-union and regular, part-time non-union employees automatically become eligible to participate on the January 1st following their date of employment.

At year end, based upon the decision by the Board of Directors may make an additional discretionary profit sharing contribution to all eligible employees.
 
Eligibility – 401(k): All regular, full-time non-union and regular, part-time non-union employees are eligible to participate and will be automatically enrolled for a minimum deferral of 4% on their date of employment. Any employee who does not wish to participate must sign a deferral waiver.

Eligible employees can contribute up to a possible maximum of 20% of eligible compensation based on total participation. 

Employees are offered a choice of investment vehicles.

Employees are always 100% vested in their own contributions.  Profit sharing money vests over a period of 6 years. Company 401k match money vests immediately.

Holidays

Great Lakes Synergy Corporation/C1 generally recognizes the following holidays on an annual basis. The actual holiday number and schedule may vary annually depending on how they fall during the year. A holiday schedule will be posted annually at the beginning of each year.

Flexible Spending Account

Maximums/Minimums
Health Care Flexible Spending Account (HFSA)
$5,000 maximum annual contribution
$300     minimum annual contribution

Dependent Care Flexible Spending Account (DFSA)
$5,000 maximum annual contribution (regardless of the number of dependents)
$2.500 maximum annual contribution for married filing separately
$300     minimum annual contribution
